Instructions on submitting application
  1. Print Application
  2. Answer all questions completely, honestly and accurately.  Make sure all your information is current.  Please use black ink
  3. Borrower and Co-Borrower sign and date application - no electronic signatures
  4. Attach all required documents ( see tab for Loan Information for the required documents)
  5. Mail to P.O. Box 6160, Chillicothe, Ohio 45601 or scan and email to info@blossertrust.org or drop it off at 60 Riverside Street, Suite 130, Chillicothe, Ohio.
  6. Please note that for security and legal verification purposes, we can only accept properly scanned or digitally generated documents.  Submissions containing photographs of documents will not be processed due to the sensitive nature of the information.
